What is the DVLA V62 form?
Form V62 is used to apply for a vehicle registration certificate (V5C log book) when the original is lost, stolen, damaged or has not arrived. It is issued by the DVLA in the United Kingdom.
It also covers cases where you have just bought a vehicle and the previous keeper has not given you the V5C, but you have proof of ownership.
If you've just bought the vehicle and you don't have the V5C, send the V62 with the green slip (V5C/2) you received from the seller.
When should you use the V62?
Use this document if you:
- Your V5C log book has been lost, stolen or damaged
- You've bought a vehicle and the V5C has not arrived
- You need to update vehicle details when no V5C exists
- You're applying for a duplicate after a name change
- You need to register a vehicle that's been off the road

DVLA V62 form: apply for a replacement V5C log book
The V62 is the official DVLA form used to apply for a replacement V5C vehicle registration certificate (log book) when the original has been lost, stolen, damaged or has not been received from the previous keeper. It is widely used by both private buyers and motor traders.
Our online editor lets you complete the V62 with all required vehicle details, keeper information and reason for the application, then download a clean PDF to print and post to the DVLA in Swansea along with the appropriate fee.
If you have just bought a used vehicle and the V5C never arrived, attach the green slip (V5C/2) given to you by the seller. The DVLA will then issue a new log book in your name and update its records accordingly.
